The plunger were
broken when assembly the handle. From the
broken parts investigation could infer that there
are several reasons to cause the issue:
1.
The vender supplied nonconforming
components:
a.
The ID (Inside Diameter) at
bottom was beyond the tolerance.
b.
Inner face was rough.
c.
Unclean chips was found inside
bottom of plunger.
2.
The "Receiving Inspection" process was
inadequate.
To ensure this will not happen again, a quality
control meetingthe was called with QC and the
vendor, and the DMAIC tool were utilized to improve
the process.

Conclusion:
1.
The vender should improve their
process to ensure the product
quality. The suggestion is:
a.
Make two special gage to
measure the ID, for both
side.
b.
Maintain the machine
feeding rate and train their
employees.
c.
Maintain the proper tool
for the cutting process and
clean chips.
2.
The QC "Receiving Inspection"
process must be improved.
